September 2025 Dance Production Participation Form

Dear Parents and Guardians,

We are beginning preparations for our upcoming Dance Student Production in September 2025.

In order to structure rehearsals and creative sections of the show, we kindly ask you to indicate whether your child will participate or not in the end of the year show.

Please also let us know whether your child will be joining our Summer Camp or continuing with Regular Classes during the summer, as this will affect show groupings and rehearsals.

Important Note

Participation in the production requires a non-refundable show fee of $75, which covers stage rental, technical costs, costumes, and other production needs.
This performance is an optional extracurricular activity and is not included in regular tuition or syllabus training.
